Release of Medical Records
To provide seamless and comprehensive care, we recommend that all primary care patients with existing medical records complete a Release of Medical Records form. This step helps us gain a full understanding of your medical history to better serve your health needs.
Once the form is submitted, our team will send the request to your current healthcare provider. Please allow 2–4 weeks for your records to be processed and transferred.
